Title: Evidence for end of life service in Community setting:

Initial funding was used to provide intensive training for community hospital staff providing end of life care. This is required for implementation of this initiative. By providing ongoing support and refining the admission process, more patients can be cared for in community hospitals.

Since the training finished, three patients have been successfully cared for in their end of life phase within this setting, exceeding the minimum target. Previously, some patients who had end of life care needs were cared for in the community hospitals because many patients already on the wards were elderly. These three people had specifically been recognised as needing end of life care rather than patients who had deteriorated rapidly and died.
